Primary Care Networks

Primary Care Networks (PCNs) are groups of GP practices working together with other local organisations such as community health services, mental health, social care, pharmacy, hospital and voluntary services. They will support the needs of a growing population that is living longer and may need to access local health services more often.

Our GP practices have been working together for a number of years through locality networks, clusters and partnerships. The NHS Long Term Plan and the new five-year framework for the GP contract, published in January 2019, made this more formal without creating new statutory bodies.  PCNs were established from July 2019.

In practice PCNs will build on the work already undertaken and the current services offered by your GP practice. It will mean greater provision of proactive, personalised and coordinated care, as well as more integration between health and social care. This will provide clear benefits for patients and GPs.

PCNs are based on GP registered lists and typically serve communities of 30,000 to 50,000. There are some slightly smaller and some slightly larger than this, as is the case in the East Lincolnshire area.  They are designed to still provide the personalised care valued by patients, but be big enough to have impact and economies of scale through closer working.

All GP practices across the country have been encouraged to be part of a local PCN, with the aim that PCNs cover the whole country. In the East Lincolnshire area we have 100 percent patient population coverage via three PCNs – East Lindsey PCN, Skegness and Coast PCN, and Boston PCN – see the drop-down menus below for more detail.
